Health History
Schedule a complimentary 15-minute consultation to assess suitability for GLP-1 (Semaglutide or Tirzepatide) medications and ask questions about our services.
If you choose to move forward you will receive intake forms, lab orders * and schedule a new patient visit.
*Can use labs that have been drawn with in 3 months.

Custom Assessment
During your new patient appointment, we determine a medication plan based on your history, labs*, lifestyle habits and
personal goals. If no contraindications, medications are prescribed and delivered directly to you with all the supplies needed for administration.*Lab results must be available prior to the new patient visit

Program Enrollment
Once you begin a treatment plan, we will schedule a 2 week follow up followed by monthly follow up thereafter. During your follow up we will review your progress, adjust medication as needed and place your monthly medication order.
